Changes to the registration process during the COVID 19 pandemic
Please download a registration form here and complete as much as possible.
If you are able please scan and return to firstname.lastname@example.org or place the form in an envelope and post through our external letterbox which is accessible at any time.
Proof of Photo ID and something that confirms your address would be beneficial not essential. This can also be emailed or posted to us with your form (please do not send originals)
To register simply visit the practice with photographic ID and recent proof of address i.e. utility bill. You will then need to fill out a short form, have your ID checked and registered on the system. Please ask if you require help in completing the form.
You will then be given an appointment with the HCA who will carry out a new patient check and record any relevant past and present medical conditions.
Your medical notes will also be sent to the practice from your previous GP. You also have the right to express a preference to which GP or nurse you preference to see either generally or in relation to a particular condition. We will record your preference for future appointment bookings.
